CONTROLLED SUBSTANCE AGREEMENT

Here's what to expect:

Complete the New Client Paperwork with your current information and history. A provider will contact you for an appointment within 48-72 hours if you are a good fit for this practice.

For NC residents only. Visits can be either in person or via telehealth.

If outside of NC, appointments must be done in the office. We will establish a relationship and set expectations for your level of care.

The provider will set aside time weekly to answer questions received via email and the portal. You will have ongoing support throughout your journey.

If approved by the provider, returning clients will have the option to complete visits via video chat or telehealth through the portal.
